Sailor Neptune returns in episode 167, the first episode of Sailor Moon Stars. Michiru and Haruka have just left the aquarium, when they are attacked by Nehelenia's mirrors. Sailor Pluto arrives, with baby Hotaru, and tells Michiru and Haruka to transform, which they do. This is the last time that we see Sailor Neptune's transformation sequence.
The enemies are strong and nearly defeat the three outer senshi, until Hotaru suddenly powers the three up and they become super senshi. The stronger senshi defeat the mirror enemies, but quickly recognize that they have a new mission.
Later in episode 170, Sailor Neptune and Sailor Mars become paired off in Nehelenia's world, after a broken mirror splits up the senshi. Sailor Neptune and Mars have a small intuition battle, a trait both are known for, and eventually Mars gives in and follows Neptune. The pair is surrounded by fire, and while Mars flips out, Neptune calming tells Nehelenia that they are not fooled by her mirages.
The fire vanishes, but Nehelenia takes Neptune captive. Mars arms her flame sniper, but Nehelenia pulls Neptune close, so that she will be hit before Nehelenia is. Stuck in a dilemna, Mars drops her arrow. Neptune pleads with Mars, telling her that one of them must stay alive to save Usagi. Mars then launches her arrow, and suceeds in hitting Nehelenia, who turns out to be only a mirror image. Both senshi pass out and are captured in the mirrors.
As the mini-series versus Nehelenia comes to a close, Neptune helps Eternal Sailor Moon to heal the Dark Moon Queen and send her back to the past for another chance to live her life the way she should have.
At this point Sailor Neptune appears only a few episodes. Towards the end of the series, however, she and her partner Uranus, appear more frequently, especially in the battle against Galaxia. In episode 196, Michiru and Haruka are the first to leave the shrine after Galaxia's invasion, and head to Galaxy TV, Galaxia's base. They meet up with Hotaru and Setsuna, who both feel that between the four strongest outer senshi, one of them would be able to defeat Galaxia. The next time we see the outers, they are walking through the television station. They arrive at a room with multiple TV screen where they witness the deaths of the inner senshi.
In episode 197, Chibi-Chibi's power allow the outers to pull Sailor Moon and Starlights from Galaxia's throne room. Sailor Uranus and Sailor Neptune head to Galaxia's throne room to defeat her. Sailor Neptune's deep submerge attack is blown away at the swipe of Galaxia's arm. She easily stops Haruka's space sword as well, cracking the blade with her bare hands. Sailor Pluto and Sailor Saturn arrive just in time to stop Galaxia from taking Neptune and Uranus's starseeds.
Galaxia, however, quickly defeats all four outer senshi. She offers them a choice, die or surrender their star seed and work for her. Sailor Pluto and Saturn immediately say they would never work for Galaxia. Uranus, however, expresses interest saying, "Working under Galaxia and becoming wild doesn't seem too bad. If I can be with you, I won't hesitate to sell my soul to the devil." Neptune is flattered and agrees. The two surrender their starseeds to Galaxia, and receive her power in exchange.
Galaxia then orders Sailor Neptune and Uranus to take the starseeds from Sailor Pluto and Saturn. Pluto asks Neptune and Uranus why they are doing what they doing. Neptune replies, "You know why," and Uranus says, "This is how we fight." She and Neptune then take Saturn and Pluto's starseeds. The two outer senshi simply close their eyes and accept their deaths. Sailor Moon and the starlights arrive just in time to see Saturn and Pluto fade away. Sailor Moon cannot believe that Neptune and Uranus are on Galaxia's side.
Episode 198 begins with Uranus and Neptune having a short conversation with Moon, in which she tells the outers to stop, they are friends. Neptune and Uranus laugh and tell Sailor Moon that they will take her starseed before she feels any pain, because they are friends. Sailor Moon tells Neptune and Uranus that she still believes in them. Galaxia then tells Neptune and Uranus to wait. She asked Sailor Moon if she still believes that the planet will be saved. Moon replies that she does, and Galaxia tells her new minions to show Sailor Moon reality. Uranus uses her new power to plunge Usagi into a dream world where she is told by her friends that the battle is over and that everything was just part of the plan to defeat Galaxia.
The dream doesn't last long and Sailor Moon awakens to find herself inside Galaxy TV, however, Moon still believes her dream was reality and rushes Uranus asking her about the plan. Uranus replies by slapping Sailor Moon. The starlights then arrive, awakened from their own dreams, and Uranus sarcastically asks them if they enjoyed their nap. Fighter tells Uranus that her power comes from Galaxia.
Neptune then steps out of the shadows, looking as cute as can be I might add and says, "Those words don't sound like you want to beat Galaxia. Do you think you can defeat Galaxia if you can't beat us." Uranus insults Fighter, who attacks Uranus and manages to land a punch in her stomach. Uranus is surprised and softly says, "You can do it alright," before punching Fighter and sending her flying across the room. Neptune and Uranus then both simultaniously attack, yelling "Deep Submerge," and "World Shaking." They practically blow the roof off of Galaxy TV, sending the starlights and Sailor Moon flying to the roof.
Galaxia reappears and tells Sailor Neptune and Sailor Uranus to take their starseeds. The two senshi lift their arms in the air, ready to follow Galaxia's commands. Suddenly their eyes turn from glazed over to normal and together they turn around and send their attack at Galaxia. Galaxia, however, does not have a starseed, and Uranus and Neptune's attack fails.
Galaxia is surprised that the senshi of Earth were strong enough to resist her control. She laughs, as if she enjoyed the deception, and removes Neptune and Uranus's braclets. Neptune and Uranus except their fate - they are going to die and they are going to hell for betraying their friends, even if for the right reasons. They fall to the floor, and Fighter yells at Uranus that they can't just leave Sailor Moon.
Uranus turns her head as she falls so that she is looking at the starlights and Sailor Moon. She says, "Fighter what you're trying to protect isn't space, peace, and the future, is it? From now on you want to protect her..."
As they fade away, Neptune and Uranus struggle to reach out and touch each other's hands. They succeed in doing so for a moment, before they vanish away, leaving Sailor Moon to cry for her lost friends.
Sailor Neptune returns at the end of episode 200 after Sailor Moon has freed Galaxia from the grips of Chaos. It is here she tells Usagi that the world was saved because of her.
